Why do you need to complete a Sales Tax Exemption From at this time and submit it to us promptly? Due to the Fall 2018 U.S. Supreme Court case South Dakota v. Wayfair Inc., we are required to collect sales tax on all taxable sales in states where we have sales tax nexus (also called "sufficient physical presence" or economic connection").  

So, if you have not already done so, please forward your Sales Tax Exemption Certificate to us via email, fax or mail.  If you are tax exempt, please fill out the appropriate Sales Tax Exemption Form and return to us via one of the following methods:

3. Mail your completed form (and also keep a copy for your records) to:
